Patient Care Coordinator

THE ROLE:

The Patient Care Coordinator – Patient Scheduler is responsible for providing sales, clerical and administrative support in order to ensure that customer services are provided in an friendly, effective and efficient manner. This person acts as a liaison between educators, support staff, medical professionals and clients to ensure all aspects of patient care are served and goals of company are achieved.

The Patient Care Coordinator reports to the Vice President, Operations and is responsible for providing outstanding customer service and support. Failure to provide these services in an efficient and effective manner will result in disruption in the provision of services.

Responsibilities

  • Call prospective patients that have been referred to the company in order to enroll them into a class
  • Answer questions about ABC and provides callers with address(es), directions, and other information
  • Provide office support services in order to ensure efficiency and effectiveness within the Home Office
  • Receive and Track returned class education packages
  • Data entry of pre and post class participant materials
  • Maintain the general filing system and file all program materials
  • Order, track, and follow up on Insurance Referrals
  • Maintain accurate filing system of patient information
  • Track Educator Introduction Calls – Complete calls when Educator is unable to make calls prior to class
  • Prepare all aspects of Patient Welcome Packets

REQUIREMENTS

Education and/or Experience:

Either One year certificate from college or technical school; or three to six months related experience and/or training; or equivalent combination of education and experience.

Language Skills:

Reads and comprehends simple instruction, short correspondence, and memos; Writes simple correspondence; Presents information in one-on-one and small group situations to customers, clients, and other employees.

Reasoning Ability:

Applies common sense understanding to carry out detailed, but uninvolved, written or oral instructions; Deals with problems involving a few concrete variables in standardized situations.

The candidate must have proficient knowledge and skills in the following areas:

  • Great customer phone skills and ability to communicate and listen
  • Computer skills including the ability to use spreadsheets and word programs at a proficient level
  • Stress and time management skills
  • Need business skills or commercial sense
  • Need interest in health, healthy living, diabetes
  • Compassionate person who cares about others
  • Preferred Skills

It would be good but not necessary for the candidate to have the following skills:

  • Any experience or familiarity with health insurance verification and/or billing
  • A background or some experience with health and/or nutrition
  • Knowledge of or experience dealing with diabetes

Salary Range: $17.00 – $20.00/ hour
